Person-centred care planning course

Course programme 3rd & 4th July 2008

SHARP Records & Plans are repeating our very successful 2 day courses, first held in 2006, which are suitable for proprietors, managers, deputy managers, nurses and senior care staff from care homes for older people. It is also relevant for care professionals involved in quality of care for care homes.

You should attend if you

Learning will be via a range of methods, with opportunities to explore how the Act will affect your approach to caring for older people in your home. All learning materials will be related directly to working with this group of people.

Aims of the course

Day One

Aims

To reflect on the real meaning of person centred care and what it means being person centred.

To use this reflection as the foundation in record keeping – developing strengths based assessments and care planning which support and enable people experiencing a dementia to live a quality life.

  • Reflecting on what being person centred means
  • Knowing what person centred care looks, sounds and feels like
  • Learning how to group the ‘ language’ of dementia and a feeling based approach
  • Avoiding labelling language in care planning
  • Interpreting the real meaning of behaviours we find difficult
  • Action planning on how to make person centred care happen.

Day Two

Aims

To acknowledge the link between ‘care-giving’ in a person-centred way and ‘record-keeping’ in a person-centred way.

To increase understanding about how to write in a person-centred way and demonstrate this by:

  • Using positive expressions rather than negative expressions
  • Writing about capabilities and not just problems
  • Writing about the level of support a person needs to achieve something
  • Not writing in generic terms
  • Recording how the person can make choices about their care according to their ability
  • Involving the person in drawing up and reviewing their care plan
  • Recording clearly if you have involved other people in the assessment and care planning and why, according to the requirements of the Mental Capacity Act 2005.

What the course will cost
The two day course is excellent value at £199 per person for the whole course (including VAT @ 17.5%). We are offering an Early Bird discount of £20 for all bookings made on or before 14th June 2008. All refreshments and training materials are included for both days.
