Centred Care & Care Planning the SHARP way
Top
tips to practice person-centred care
- Call the person
by their name, not ‘darling’ or
sweetheart’ – anyone can be called by these names. Using
the person’s name emphasises the uniqueness of them as an individual
and helps them know you are talking to them.
-
Always talk with the person, include them in conversation, even if
there is no reply
- Assume the person can understand everything you say
- Have good knowledge
of the person’s life history, as much as
they feel comfortable in revealing, so that you can show an understanding
of them as a unique person
- If
the person is not able to tell you about themselves, then involve
relatives, friends and advocates, where appropriate,
to find out
about the person, to find out how the person would like to be cared
for.
- Do not use generic
derogatory terms, eg ‘the feeders’ or ‘doing
the toileting’.
