Top
tips to care planning and writing in a person-centred way
- Write clearly how the person wants to be addressed and ensure all
staff use this name at all times
- Use positive
expressions rather than negative expressions, eg ‘mobilises
in a wheelchair’, rather than ‘wheelchair bound’
- Write what the
person can do, rather than what they can’t do,
eg ‘is able to wash hands and face’, rather than ‘cannot
wash themselves completely’
- Write what level
of support a person needs to achieve something, rather than that
they can’t do it, eg ‘needs support from one
carer and his walking frame to walk’
- Do not write
generic derogatory terms, eg ‘uncooperative’, ‘a
wanderer’ or ‘a kicker’
- Record clearly
how the person can make choices about their care according to their
ability, eg ‘Offer a choice of two items of clothing,
which are suitable for the weather and the occasion’
- Record clearly
what involvement the person has had in drawing up and reviewing their
care plan, eg ‘Mr Smith helped to compile his
care plan by giving information about what he likes and dislikes and
what support he needs to live his life to the full’
- Record clearly if you have involved other people in the assessment
and care planning and why, according to the requirements of the Mental
Capacity Act 2005.
