Person Centred Care & Care Planning the SHARP way What is Person Centred Care? The term makes it sound obvious, but that’s not the case. Of course the literal meaning gives a clue to some of the features, for example, focusing on the individual when care is planned and carried out, but that’s not the whole story. Usually the term person-centred care is linked with the care of people with dementia, but the concept is applicable to all people needing care, even if the main aspects of care are physical.That's because we all have a psychological aspect to our needs and this approach addresses those needs.
