A
key feature
of SHARP Records & Plans is the Support Package. The SHARP Records & Plans system
is not
just
about
pieces
of paper - the paper is a means to an end, not
an end in itself.
The
SHARP Records & Plans "set-up" is:
- An
initial visit to the home to introduce the SHARP Records & Plans system
to the Manager and staff.
- 3
days education and training for 2 link staff, in a group setting
with other users. The focus of these sessions is to
- Update
knowledge in key areas of care.
- Explore
what needs to be recorded
- Practice
setting up and writing a service user plan using SHARP Records & Plans
- A
follow-up visit to review progress to help and advise
Ongoing
support for SHARP Records & Plans after the first year is
provided through:
- An
annual visit to the home to provide updated training to the staff
and
- audit
service user plans to measure progress
- answer
questions and provide support
- An
annual study day for link staff, in a group setting including:
- clinical
updates, with guest speakers on relevant specialist subjects
- specific
help and advice with maintaining care records
- peer
support & discussion
